Outcome of Endoscopic Third Ventriculostomy (ETV), our Early Experience

Abstract:
Endoscopic Third Ventriculostomy (ETV) is a relatively newer technique and is well known for its reasonable clinical and radiological outcome 1. It`s a treatment of choice for obstructive hydrocephalus 2. It is gaining more and more popularity, especially amongst the emerging neurosurgeons. It’s an alternative procedure, to Ventriculoperitoneal shunt, in which a bypass is created on the floor of third ventricle 3. As shunt placement improves neuropsychological functioning, especially in verbal and visual memory, attention and cognitive flexibility 4. Authors have recently started Endoscopic Third Ventriculostomy procedure in their institute along with evaluating the cognitive function, and are sharing their early experience. So far, 30 cases have been done with relatively moderate outcome. To evaluate the outcome of Endoscopic Third Ventriculostomy in Obstructive Hydrocephalus and cognitive functioning. It’s an ongoing study and so far 30 cases are included in the study. Out of these 30 patients, 23 belonged to pediatric age group and the main cause for obstructive hydrocephalus was aqueductal stenosis along with myelomeningocele while 7 patients belong of the adult age group and in these patients the cause of obstructive hydrocephalus was brain tumor. The outcome was measured on the basis of mortality, post operative recovery, relief of clinical symptoms and improvements in the cognitive behavior. Uptil now, 30 patients were included in this study. The rate of in-hospital mortality was 3.3 % which was a little higher according to literature. A total of 23 (76.6 %) patients were discharged on 2nd post operative day. 1 patient expired on 3rd postoperative day, so the in-hospital mortality rate was 3.3%. 6 patients developed post operative complications, 3 of them develop wound infection, 1 developed meningitis and 1 patient developed seizures post-operatively. They were managed accordingly and were discharged satisfactorily without any morbidity. We can conclude so far that in expert hands endoscopic third ventriculostomy is reasonably good procedure with moderate clinical outcomes and have positive cognitive improvement.
