Topical management of burns.

Abstract:
The commonest topical agent used on the burn wound was Vaseline. Ordinary dressing gauze impregnated with veseline was the method employed. The next most common topical was gentamicin (Genticyn, Nicholas) used in the form of 0.1% cream. It was employed in 49 cases, including 3.0% indoor and 4.3% outdoor cases. The cream was used either by application (two to three times a day) on exposed burns or it was employed as a layer spread on dry guaze followed by a dressing changed once daily. 90 cases were treated with 0.1% nitrofurazone cream (Furacin, SKF). The cream was employed in a similar fashion to that used for gentamiein, either with wounds exposed or with dressings changed daily. Mafenide acetate 10% cream (Sulfamylon-Winthrop) was used in 42 cases, 41 of which were outdoor and only one indoor. Open treatment gave very significantly higher survivals than controls (01>p>0.0001). Mafenide acetate and gentarnicin also gave some reduction in mortality (0.1>p>0.05) while nitrofurazone, framycetin and saline dressings did not prove to be significantly different from vaseline controls in respect of mortality rates. Open treatment proved successful and is rather remarkable, particularly because no isolation and no environmental control were needed.
