Retrospective Evaluation of Galactomannan and (1?3) ß-d-Glucan Assays as Diagnostic Tools for Intracranial Fungal Infection in Tertiary care Centre of Karachi

Abstract:
Fungal infections of the central nervous system either alone or as an intracranial extension, have been increasing for the last ten years, the top fungal species being yeast, moulds, and filamentous fungi. The invasion occurs either through the blood-brain barrier or the paranasal sinus, which are sites for Candida and Aspergillus, respectively. The Galactomannan and 1, 3 B-D-glucan assay have advanced a position as diagnostic criteria, being an essential component of the cell wall of fungi. Our study belongs to one of the semitropical areas of Asia, where fungi thrive well. Twelve reckoning patients, three immunocompromised and nine with no significant past medical history, were analyzed based on biomarkers, radio imaging, and CSF analysis. Suboptimal GM and high titer of BDG in most cases reflect the specificity towards invasive mycoses. Moreover, the CT scan results were indicative of fungal sinusitis, while MRI showed relevancy to brain parenchyma. In reflection, the Fungal Biomarkers also aided in distinguishing between any neoplastic involvements where they would be registered negative. The above modalities together with the clinical approach are guaranteed to make the detection uncomplicated. Though there is still an obligation to analyze the gap in diagnostic approach due to patient noncompliance to follow-up or limited facilities to help both the patient and the paramedic personnel for quick recognition of cases and timely treatment.
