Accuracy of resistive index and pulsatility index in preoperative characterization of benign and malignant ovarian masses.

Abstract:
OBJECTIVES: To evaluate the diagnostic accuracy of Doppler Sonography in preoperative characterization of benign and malignant tumours of the ovary by taking Resistive Index (RI) and Pulsatility Index (P1) values and comparing it with the histopathology of the mass. STUDY DESIGN: A Cross-sectional comparative study PLACE AND DURATION: Department of Obstetrics and Gynaecology Unit - III Lady Willingdon Hospital Lahore (King Edward Medical st th University) for one year duration from 1 January 2014 to 31 December2 014. METHODOLGY: Total of 220 patients presented with ovarian masses having a clinical suspicion of malignancy were included in the study. Clinical examination was done to evaluate the texture of the lesion. Gray scale ultrasound was performed followed by Doppler ultrasound to observe the blood flow in the lesion and to calculate Resistive index (RI) and Pulsatility index (Pl).After removal of the mass, histopathology was done for all the cases. RESULTS: On comparing histopathological and Doppler sonography findings, 54.54%patients were positive and 45.45 % were negative of having malignancy on both diagnostic procedures. There were 13.33% patients who were positive on sonography but were negative on histopathology, while 11.66% patients were negative on Doppler but were positive on histopathological diagnosis. Sensitivity, specificity, positive and negative predictive value of Doppler sonography(by the values of RI and PI was 90.91%, 86.21%, 88.24% and 89.29% respectively with overall diagnostic accuracy 88.71%. CONCLUSION: This study proved the efficacy of resistive index (RI) and Pulsatility (PI) value on Doppler ultrasound as a method of choice to characterize preoperatively as benign and malignant ovarian lesions.
