Treatment of Old Fractures of the Lateral Condyle of Humerus in Children

Abstract:
Objective: To study the outcome of open reduction and fixation with Kwires of fractures of lateral condyle of humerus in children presenting between 3 - 12 weeks after injury. Methodology: This was a retrospective study at orthopedic complex in Bahawal Victoria Hospital for 3 years from March 2008 to February 2010. We included 18 patients (15 boys, 3 girls, mean age 7.2 years; range 411years) with late presentation of fracture of the lateral condyle of humerus. Four patients presented between 3-4 weeks, 8 between 5-8 weeks and 6 patients between 9-12 weeks. Fractures were classified 8 as stage II and 10 as stage III. All patients were treated by open reduction and internal fixation with k-wires with or without bone grafting. The results were assessed by the criteria of Agerwal et al with little modification. The average duration of follow-up was 15 months (range: 12-24 months). Results: The four patients presented at 3-4 weeks post injury showed excellent to good results. In 8 patients presented at 5-8 weeks, the results were excellent in 1, good in 3, fair in 3 and poor in 1 patients. In patients presented at 9-12 weeks, the results were fair in 3 and poor in 3 patients. Conclusion: Open reduction and internal fixation is an effective treatment in fractures of lateral condyle of humerus in children presenting between 3-12 weeks after injury.
