Abstract:
To evaluate various histological prognostic markers on biopsy material of urothelial carcinoma of the urinary bladder a prospective and non-interventional study was carried out in Department of Histopathology, Armed Forces Institute of Pathology, Rawalpindi. The study included 97 biopsy specimens of transitional cell carcinoma (TCC) of urinary bladder diagnosed at the department of histopathology during a period of two years. Various histological prognostic parameters were assessed including tumour grade, tumour margins, inflammatory response, muscle invasion, vascular lymphatic invasion, Squamous differentiation and abnormality in adjacent bladder mucosa, Out of 97 urothelial carcinomas, 4 (4.2%) tumours were grade I, 58 (59,8%) were grade II and 35 (36.0%) were grade III TCCs according to the WHO Classification 1972. Thirty-nine (40.2%) TCCs revealed infiltrating margins whereas 58 (59.8%) had pushing margins. Tumour infiltrating lymphocytes (TIL) were absent in 51 (52.6%) cases, moderate in 21 (21.6%) biopsies whereas 25 (25.8%) cases had extensive lymphocytic infiltrate. Muscularis propria was included in 53 biopsies of TCC and invasion was seen in 25 (47.1%) of these tumours. Vascular lymphatic invasion was present in 32 (32.9%) TCCs. Forty-two (43,2%) TCCs revealed squemous differentiation of varying degrees, It can be concluded that most of the urothelial carcinomas of urinary bladder seen in our material were grade II and grade III TCCs, Infiltrative margins, dense lymphocytic infiltration, vascular and muscular invasion and squamous differentiation were seen more frequently in grade III than grade II TCCs.
