Infant Botulism: A diagnostic predicament in Pakistan

Abstract:
Infant botulism is an intestinal toxemia. It is a well-known disease which results when bacterium Clostridium botulinum spores are swallowed which causes botulinum neurotoxin in intestine. The neurotoxin binds to cholinergic nerve terminals and cleaves intracellular proteins necessary for acetylcholine release, resulting in bulbar palsies, hypotonia, and a symmetric, descending, flaccid paralysis. In children it is rare but is a cause of paralysis. In recent year 59.1 % children in Pakistan have shown botulism. The diagnosis of botulism requires proper conformation from laboratory tests. The laboratory tests botulinum toxin in serum or isolation of the C. botulinum from stool/gastric aspirates, But because of unavailability of technical facilities in Pakistan. Electrophysiological studies are proven to be the best diagnostic tool. Which is quick and economical. It helps to reduce morbidity, mortality and length of stay in hospital.
