Therapeutic effects of inhaled Nitric oxide in postoperative patients of heart disease with pulmonary hypertension.

Abstract:
Background: Inhaled nitric oxide selectively dilates pulmonary vessel. This study was performed to assess therapeutic effects of inhaled nitric oxide in postoperative patients of heart disease with pulmonary hypertension. Methods: Twenty postoperative patients of ventricular septa) defect and twenty postoperative patients of mitral valve stenosis with pulmonary hypertension (mean pulmonary artery pressure >2.67KPa) were enrolled in our study. Parameters in hemodynamics and oxygenation were measured with Swan-Ganz technique, 5min before nitric oxide inhalation, 5,15 and 30min following nitric oxide 20ppm (per part million) inhalation, 15min after stopping nitric oxide 20ppm inhalation, 15min and 5hour following nitric oxide 6ppm inhalation and 15min before and after weaning from mechanical ventilator respectively. Result: In all patients, compared with those at 5min before nitric oxide inhalation, pulmonary artery pressure, pulmonary vascular resistance, alveolar arterial oxygen difference and intrapulmonary shunting significantly decreased, but arterial oxygen partial pressure, oxygen content and oxygen availability index significantly increased at 5,15 and 30min after nitric oxide 20ppm inhalation (p0.05). Conclusions: The persistently inhaled low dose nitric oxide is an effective and safe method to the postoperative patients of heart disease with pulmonary hypertension.
