Topical and systemic antibiotic prophylaxis in neurosurgery

Abstract:
To study the effects on the neurosurgical wound infection rate of combining the use of parenteral antibiotic prophylaxis with a modification of the Malis technique of topical wound irrigation with an antibiotic solution. Place and Duration of study: The study was conducted at the Neurosurgical Centre, Combined Military Hospital, Rawalpindi, over a period of eight months, from January, 2000 to August, 2000. Methods: The incidence of wound infection was studied in a series of 544 clean, consecutive neurosurgical operations. The surgical techniques employed were similar. All the patients received cefuroxime 1.5 gm intravenously (dose adjusted for children according to body weight) at the time of induction. The operative wounds of all patients were irrigated by a solution of 500 mg of cephradine in 20 ml of normal saline towards the end of the operations. In the first group of 288 patients two additional doses of cefuroxime 750 mg intravenously at 8 hourly intervals were given postoperatively. The second group of 256 patients did not receive any postoperative antibiotics. Results: In the first group of patients, 03 cases (1.04%) of postoperative infection were encountered. In the second group of patients only 01 case (0.39%) of postoperative infection was observed. Conclusion: It was concluded that addition of wound irrigation with an antibiotic solution to parenteral antibiotic prophylaxis lowers the infection rate considerably. Moreover, a pre-operative dose of prophylactic antibiotic is equally effective as continuing the drug postoperatively
